About The Position

Home Care Network (HCN) is a leading Medicare-certified home care and home health agency providing total patient & family support for children, adults, and seniors. Additionally, we have a strong relationship with the Ohio Department of Developmental Disabilities (DoDD) as well as a diverse team of caregivers, as we employ dozens of family caregivers, DSPs, and former Independent Providers who find the ease of compliance and reliability of income through our agency a greater advantage than going it alone. We are seeking skilled, compassionate, and professional Direct Support Professionals (DSP) and current/former Independent Providers with active DoDD Certification to join our home health agency and provide in-home care and support to existing and new DoDD patients in Southern/Southeast Ohio communities and surrounding areas. This is a long-term employment opportunity that can be built to fit your schedule - whether that's: as a 2nd job (working around your primary schedule) as a consistent Part-Time position starting at Part-Time and increasing to Full-Time in the first 30-60 days YES : if you currently care for a family member or loved one, we are accepting new clients! Friendly travel expectations: Our standard travel radius for client coverage is 15-30 minutes from every employee's home. DSPs require DoDD Certification. Candidates with a valid DoDD DSP certificate within the past 12 months will be prioritized, however we will provide training to certify the right candidate committed to quickly secure required certification if needed. HHA, CNA, or STNA a plus but not required. Familiarity with Ohio Board of DoDD rules is required for DSPs / a plus for all other Caregivers. 1+ year(s) caregiving experience required + 1+ year(s) DSP / DoDD experience preferred. 1+ year(s) in-home care / home health experience highly preferred.

Requirements

  • DoDD DSP Certificate: Candidates must have either a valid DoDD DSP certificate within the past 12 months, or willingness to immediately complete DoDD training through our agency.
  • 1+ year(s) caregiving experience required
  • Employees must meet all training and/or certification requirements of the State of Ohio. This includes specifications of contracted agencies for which services are being provided and successful completion of a formal certification training program or a written skills test and competency evaluation required.
  • Active CPR and First Aide Certifications required (or can be renewed prior to hire date through our agency or accredited third parties).
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed is required.
  • Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
  • Strong attendance and dependability track record with previous employers.
  • Good interpersonal skills and compassion for helping others live their best lives are required.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
  • Be at lease eighteen (18) years of age.
  • Current driver's license / State ID and reliable transportation are required.
  • Willingness and ability to pass a background check.

Nice To Haves

Responsibilities

  • Deliver direct patient care and support, following the established plan of care.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assists patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications.
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
  • Know and follow infection control policies including proper hand washing techniques.
  • Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
  • Document care provided and complete all forms required for the client's records.
  • Complete the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
  • Demonstrate safe practice in the use of equipment, avoiding equipment use until orientation has been provided.
  • Notifying supervisor(s) of educational needs immediately.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested and through approved channels in a very timely manner.
  • Treat patients and families with respect.
  • Maintain confidentiality in all aspects of the job.
  • Accept and fulfill assignments with our agency.
  • Exercise good judgment in accepting assignments.
  • Participate in required in-service programs to meet compliance requirements.
  • Identify learning needs to agency supervisor.
  • Perform other related duties and responsibilities as deemed necessary.
