About The Position

Skills Group Facilitator The Skills Group Facilitator is the primary driver of daily programming and participant support within the Community Skills Group. This role facilitates engaging group activities for adults with developmental disabilities while supporting staff in aligning programming with each participant’s Individualized Service Agreement (ISA) goals. The primary responsibilities include, but are not limited to: Facilitate daily group programming, including arts, life skills, wellness, and community-based activities Plan and prepare activity materials, guides, and instructions in advance Adapt activities to meet a range of participant abilities and support needs Support participants in building social skills, communication, and community connections Coach and support DSP staff in aligning activities with ISA goals Maintain knowledge of ISA domains and participate in ISA meetings as needed Develop and maintain monthly activity calendars Manage program supplies and inventory within budget guidelines Maintain a clean, organized, and welcoming program environment Attend team meetings and communicate regularly with leadership regarding participant progress and program updates Complete required documentation, including attendance and progress notes Follow all NKHS policies, HIPAA regulations, and reporting requirements

Requirements

  • High school diploma or equivalent required
  • Minimum of two years of experience working with adults with developmental disabilities or in a related field
  • Valid driver’s license and reliable transportation required
  • Ability to pass required background checks

Nice To Haves

  • Associate’s or Bachelor’s degree in Human Services or related field preferred
  • Experience facilitating group programming or activities preferred
  • Knowledge of ISA processes or HCBS programming preferred
  • Strong organizational, communication, and interpersonal skills
  • Ability to adapt to a variety of needs and support levels
