About The Position

On Lok PACE We are a non-profit committed to our participants. Dedicated to The Care of Elders: PACE ( P rogram of A ll-Inclusive C are for the E lderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible. Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services. POSITION SUMMARY: Under the direction and supervision of the Transportation Coordinator, the Driver will provide transportation services to all participants and make any deliveries and pickups as needed or assigned.

Requirements

  • Licensed California Driver with good driving record.
  • Ability to operate all vehicles of On Lok has some mechanical ability and understand the maintenance program for vehicles.
  • Flexibility and creativity; gets along well with frail elderly people, and exercises mature judgment.
  • Understands and carries out oral and written directions.
  • Familiarity with traffic routes and driving regulations in San Francisco.
  • Ability to lift 50 pounds.

Nice To Haves

  • One-year experience working with the frail or elderly population preferred.
  • Bilingual English/ Spanish or Cantonese/ English preferred.

Responsibilities

  • Safe operation of vehicles (includes proper use of wheelchair tie down, use of lift, etc.) while transporting participants from their homes to the Day Health Centers and community appointments.
  • Ensure participant and vehicle safety (van maintenance, pre-trip inspections, use of seatbelts, body mechanics, transfer training, and wheelchair safety).
  • Provides timely and safe transport of participants, meals and supplies as scheduled or requested (between home and center, and community appointments)
  • Supports participant's care plan as developed by IDT (assist participant in transport; make observations and report to team)
  • Other related duties as assigned.
