Organization: Hematology/Stem Cell Clinic Job Summary: This position is responsible for coordinating the clinical and logistical processes of stem cell donors. This position works 4- 10 hour shifts or 5 - 8 hours shifts within the Hematology/Stem Cell Transplant Department. There are no weekend or holiday requirements. Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the care coordination model. This position Improves outcomes by reducing all cause hospital readmissions and coordinating episodes of care among patients in a defined population or disease process. Participates in identification of appropriate patients; encourage patient and family engagement in self-care management; promote warm handovers to the next level of care by providing timely, pertinent information in a standardized way; conduct patient and family education on key elements of the patients' personal care plan by using the "teach-back" methodology and follow up phone calls; and assists the patient in navigating the healthcare system. . We perform more stem cell transplants than almost anyplace else in the nation. Cellular therapies treat certain life-threatening diseases. They are a type of immunotherapy; they help the immune system recognize and fight cancer cells. Stem cell transplantation (bone marrow transplant) is a type of cellular therapy. Stem cells are found in marrow, inside bones. They make cells that circulate in the bloodstream: red and white blood cells and platelets. A stem cell transplant treats blood cancers and some non-cancer conditions. Transplanting healthy stem cells into a person with diseased bone marrow restores the body’s ability to grow healthy blood cells. We are nationally recognized for our stem cell transplant programs to treat blood cancers.
