Lawrence Community Health Services (LCHS) brings together an interdisciplinary team of nurses, social workers, therapists, and a Medical Director who works collaboratively to support patients and families throughout our community. The Documentation Improvement Specialist plays a key role in ensuring accurate, high‑quality, and compliant clinical documentation across both Home Care and Hospice. Under the supervision of the Director of Patient Services, this role pre-codes referrals, reviews admission documentation, approves Plans of Care, collaborates with clinicians to improve accuracy, and supports weekend Intake operations. The specialist provides guidance, education, and feedback on OASIS and HOPE accuracy, regulatory requirements, and documentation best practices. This role supports key quality and compliance initiatives while serving as a clinical resource to interdisciplinary staff. This is a hybrid role, based at Lawrence Community Health Services, with a combination of on‑site and remote work as appropriate. This is a full-time day shift opportunity that includes Weekends: Saturday: 8:00 AM – 2:00 PM and Sunday: 8:00 AM – 4:00 PM, On Weekdays (Hybrid): Monday: 10:00 AM – 5:30 PM, Tuesday: 10:00 AM – 6:00 PM, and Wednesday: 10:00 AM – 6:00 PM. Days Off: Thursday & Friday.
