About The Position

Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, and outpatient care. Position Summary: The Physical Therapist evaluates and treats patients following physician referrals. He/she also assumes administrative duties as delegated by the Director of Rehabilitation.

Requirements

  • Graduate with a Doctorate of Physical Therapy from an accredited DPT program
  • Current PT licensure in the State of Illinois
  • Some experience in Physical Therapy is preferred
  • BLS preferred

Responsibilities

  • Performs evaluations, daily visits, re-evaluations, progress notes, and discharge notes on both inpatients and outpatients.
  • Communicates findings with PTA(s) following the patient’s care.
  • Initiates and updates plan of care as changes occur.
  • Charts observations and interventions, completing documentation in a timely manner.
  • Implements the patient care plan through providing and delegating direct care.
  • Identifies and communicates unusual or abnormal assessments of patient care problems to the physician and/or Rehab Supervisor.
  • Communicates pertinent patient information to the referring physician.
  • Gathers information from interdisciplinary sources to develop a plan of care.
  • Utilizes opportunities during the delivery of treatments to teach and reinforce teaching to the patient, family members, and/or significant others.
  • Reviews instructions with patient, family, and/or significant others before transfer or discharge, and sends appropriate forms with patient.
  • Follows up on customer complaints as quickly as possible by identifying steps to correct problem, communicating with coordinator/supervisor, implementing plan, and providing feedback to customer.
  • Is willing to be flexible to meet patient care needs.
  • Provides quality care in compliance with the APTA Code of Ethics
