An overview of this position entails…This position is responsible for coordinating and providing Durable Medical Equipment (DME) and supply ordering, repair, and maintenance in collaboration with health services, case management, leadership, outside vendors, and medical offices. Also responsible for maintaining the LAP tracking and ordering program, as well as incontinence and oxygen supplies.

Requirements

  • High school diploma or General Education Degree (GED) and three years of related work experience.
  • Minimum of 21 years of age required.
  • Valid Colorado Driver’s License (required)

Responsibilities

  • Demonstrate ethical and professional behavior that aligns with the values of Hilltop’s Life Adjustment program, including Service 1 how we treat the residents, and Service 2, how we treat each other. Service 1 and Service 2 is demonstrated by setting boundaries, being predictable, consistent, kind, and transparent.
  • Conducts oneself as a leader by taking ownership of the program and providing quality of care; commits to the betterment and growth of BI Services by offering solutions, developing processes, promoting communication with others, supporting communication from staff to management, prioritizing tasks, and actively participating in problem resolution.
  • Provide quality customer service to internal and external customers of the Life Adjustment Program.
  • Manage resident DME by performing monthly inspections/safety checks, coordinating necessary repairs/maintenance, and coordinating/tracking equipment orders.
  • Manage resident oxygen supplies including monthly cleaning of filters, changing tubing/cannulas/masks, and maintaining adequate supplies.
  • Order and organize resident incontinence supplies, wipes, and urinals.
  • Order and stock personal protection equipment (PPE) for “outbreak” and “colonized” infectious locations.
  • Coordinate phone calls to and from providers, vendors, families, and other interested parties to communicate changes to DME, therapy evaluations related to DME, oxygen, incontinence supplies, other resident-related supplies.
  • Work with vendors to ensure resident supplies are billable through insurance and meet the usage of the resident. Coordinate with program finance staff for all private pay needs.
  • Update tracking sheets and add new order information.
  • Provide training to staff with new DME, as needed.
  • Transport and accompany residents to medical appointments to provide resident history and support with communication, orientation, care, and transfer assist with appropriate equipment on site.
  • Report data via electronic reporting system including notes, resident summary template (RST), and care plan.
  • Act and function as part of the multi-disciplinary team (MDT).
