About The Position

TMI is dedicated to assisting children and adults with intellectual and/or developmental disabilities such as autism spectrum disorder, Down syndrome, epilepsy, and cerebral palsy, as well as the Deaf and Hard of Hearing, to live, work, and participate in their community. Requirements

  • High School Diploma; Bachelor's Degree preferred
  • 2 years working in social services or education with bachelor’s degree preferred OR 3 years working in social services or education with associate’s degree preferred OR 4 years working in social services or education without postsecondary education
  • 3 years driving experience, if applicable

Responsibilities

  • Manage caseload to include maintenance of confidential client files, monitoring of monthly attendance, and ensuring TMI database is accurate and current.
  • Provide direct support to clients when needed (sick days, vacation, etc.).
  • Engage potential employers and/or volunteer sites to develop relationships that may lead to future employment opportunities.
  • Identify, recruit, and hire direct staff when the need arises.
  • Develop and implement annual plan, goals, and objectives.
  • Schedule and participate in client-required meetings (30 Day, semi-annual, annual, Individualized Personal Plan (IPP) etc.).
  • Complete monthly client billing for services provided.
  • Complete and submit monthly progress reports to Department of Rehabilitation and/or the Regional Center.
  • Complete monthly and quarterly reports for Director of Supported Employment.
  • Develop individualized job search plans and related materials.
  • Participate in the development of a circle of support, both paid and unpaid, to build relationships with family, friends, neighbors, and other community members.
  • Develop and expand client’s participation in integrated community activities.
  • Support client’s choices and interests by identifying and implementing preferred activities.
  • Serve as a liaison to client’s family, the Regional Center, and other public service agencies.
  • Provide program training and guidance to supervised employees.
  • Review and approve supervised employees’ paper and online timesheets.
  • Complete and conduct performance reviews of supervised employees.
  • Ensure progress notes, financial tracking (if applicable), and medical appointment logs (if applicable) are completed by supervised employees.
  • Review and file such documents in client’s confidential files according to deadlines set by supervisor.
  • Assist HR Department with contacting supervised employees to obtain renewable documents and complete required trainings.
  • Complete required documentation including quarterly reports and incident reports according to established deadlines.
  • Attend mandatory meetings and trainings including all staff meetings, management meetings, team meetings, CPR, First Aid, and AED certification, and Relias (web-based learning).
  • Enter personal timesheets and mileage reimbursement into web-based systems according to deadlines.
  • Maintain communication with supervisor and members of the clients’ planning team.
  • Notify supervisor immediately if unable to work during your typical business hours/schedule (includes calling in sick). Any change to your monthly projected schedule should be documented and submitted to your supervisor.
  • Notify supervisor immediately of changes to client’s schedule and major issues such as client’s health and safety.
  • Respond to supervisor’s calls/text messages/emails promptly (within 24 hours). Urgent calls from supervisor must be responded to immediately.
  • Respond to calls/text messages/emails from clients or members of the clients’ planning team within 24 hours or the next business day if it falls on a weekend.
  • Respond promptly to emergency calls regarding clients during non-traditional working hours.
  • Drive personal vehicle to clients’ homes, place of employment, and other community resources as needed. Some positions may require use of public transportation or paratransit services.
  • Provide proof of valid driver license, annual vehicle inspection, vehicle insurance, and vehicle registration (applicable only to Temecula or Corona-based employees) throughout employment.
  • Comply with federal, state, and/or local reporting and disclosure requirements.
  • Comply with HIPAA laws and TMI policies and procedures regarding confidentiality of information.
  • Support TMI sponsored events whenever possible.
  • Other assignments and duties as deemed appropriate by the supervisor.
