We are seeking a Diversion Family Service Coordinator to join our team. The Diversion Family Service Coordinator is responsible for assisting families who are experiencing a housing crisis through diversion services, shelter assessments, and direct support. This role helps stabilize families, prevent shelter entry when possible, and ensure smooth access to shelter when needed. This position r eports to the Service Entry Program Manager. Family Promise of West Michigan Core Values: Has Passion for Mission Prioritizes Accessibility, Belonging and Community Is Dedicated to Hospitality Collaborates Intentionally Is Hungry, Humble, and Smart Assumes Good Intent Strives for Excellence and Sustained Impact Our Ideal Candidate will get to: Advocate and provide direct diversion, assessment, and crisis-resolution services to assist families in navigating their housing instability with the goal of preventing shelter entry or ensuring safe and timely shelter placement by: Conducting diversion screenings to identify immediate, safe alternatives to shelter and developing short-term housing resolution plans. Evaluating families’ access to basic needs, including; housing, income, health, education, transportation, and childcare - and coordinating referrals to community resources. Maintaining accurate, timely documentation across internal and statewide databases to support grant requirements and program outcomes. Utilizing trauma-informed, strengths-based, and evidence-based practices in all family interactions. Coordinating communication and relationships with landlords, community partners, shelters, and service providers to support housing stability. Completing shelter assessments and intakes at multiple Family Promise shelter locations, as needed. Assisting walk-in families at the administrative building with crisis support, diversion conversations, and resource connection. Other duties as assigned.
