About The Position

The Royal Wolverhampton Planned Care Teams are highly motivated and passionate about providing high quality, evidence based care to patients’ in their own homes. Possessing a ‘can do’ attitude and a commitment to improving the way healthcare is delivered. We need exceptional individuals with a passion for delivering high quality care with a focus on patient safety. You must be a Registered Nurse. The District Nursing Specialist Practitioner Qualification or relevant degree e.g. palliative care, wound care, chronic disease is desirable. You will be IT literate and be confident utilizing applications to enable remote consultation with patients/to attend MDTs. If you think you have what it takes or you are looking for a change and want to work more autonomously in a challenging environment, with opportunities for further career development then the community could be for you. The staff work closely with members of the wider multi-disciplinary teams including local Hospices, Urgent care team, Care Coordination Hub, Ambulatory Teams and Support Care Teams. Utilise clinical proficiency in delivering a high standard of nursing care. Provide managerial and leadership skills to support the Senior Sister / Charge Nurse and nursing team to deliver continually improved standards and efficient and effective use of resources.
