Job Summary The District Medical Director (DMD) will work in conjunction with and report to the Territory Medical Director. The DMD will serve in a dual leadership and clincial role, providing medical oversight for employed or contracted clinicians within an assigned district while maintaining active clinical responsibilities. This role is accountable for ensuring high quality, patient centered care, aligned with evidence-based standards and the principles of Value-Based-Care. The DMD will partner closely with clinical and operational leadership to mentor clinicians and drive improved clinical performance, continuous improvement, and accountability in care delivery. Through this work, the DMD fosters a culture of continuous learning, professional growth and accountability amongst clinicians, resulting in an enhanced patient experience and measurable improvements in health outcomes.

Requirements

  • Must have a Medical Degree from an accredited medical school or DO school.
  • Must have an active unencumbered state of Florida Medical License.
  • Must have National Board Certification/Eligibility in Family Medicine or Internal Medicine.
  • Experience in Medicare Advantage Risk and Value-Based Care Management.
  • Experience serving in a District Medical Director or comparable physician leadership role with responsibility for overseeing clinician performance, clinical quality, utilization management, and Value-Based Care outcomes across multiple sites.
  • Profeciency in Electronic Medical Records, Excel, Word, and other IT systems.
  • Skilled in oral and written communications with associates, patients, and other individuals in a professional and courteous manner.

Responsibilities

  • Maintain active clinical practice in accordance with organizational expectations.
  • Model high-quality, patient- centered care aligned with the principles of Value-Based Care.
  • Ensure adherence to CanoHealth’s Clinical Pathways aligned with evidence-based medicine and standards of care.
  • Work closely with the CDI teams to ensure quality and accuracy of medical record documentation following all CMS guidelines.
  • Document all patient conditions in a timely and accurate manner to ensure clear reflection of patient health status.
  • Ensure timely completion of all patient records to ensure patient safety and continuity of care.
  • Work closely with CanoHealth’s Quality Department to ensure all preventive measures are met.
  • Be familiar with all clinical and operational dashboards to track and trend all KPIs which guide clinical performance and quality of care.
  • Work closely with CanoHealth’s Value-Based Care/Referrals Department to ensure familiarity and adherence to specialty networks.
  • Ensure that all chronic and acute patient visits are conducted timely.
  • Be familiar with and support company initiatives as it relates to Annual Wellness, New Patient Visits, and Transitions of Care.
  • Work in conjunction with the Care Management Department to ensure that identified patients are seen and that care plans are patient centered in accordance with Value-Based Care principles.
  • Collaborate with clinical and operational staff to ensure patient medical needs are met across the continuum of care.
  • Participate in quality assurance activities related to quality of care, clinical documentation, coding accuracy, and compliance with CMS.
  • Engage in utilization review processes to support appropriate billing, medical necessity, and Value-Based Care outcomes.
  • Provide direct clinical oversight of APRN’s and Physician Assistants.
  • Oversee and provide direct leadership to clinicians within an assigned district.
  • Serve as clinical leader and educator providing coaching, feedback and professional development to support clinical excellence and engagement within the assigned district aligned with Value-Based Care and Population Health Management.
  • Work in conjunction with the VP of Quality Care and Medical Integrity and Territory Medical Director to identify clinicians needing clinical guidance and relevant continuing medical education as determined through medical review process.
  • Serve as educator as it relates to all clinical and operational dashboards which illustrate all KPIs to track and trend clinical outcomes.
  • Provide education as it relates to CanoHealth’s financial relationships with all Health Plans.
  • Foster a culture of collaboration, accountability and continuous learning.
  • Conduct monthly district level meetings with clinicians and operational partners to review scorecards and KPIs as it relates to clinical documentation, hospital admissions, transitions of care, referral management, quality gaps, patient visits, patient satisfaction surveys, new enrollments, and create individualized improvement plans.
  • Actively engage in onboarding of new clinicians with hands-on clinical training, ensuring they receive direct instruction, and support to reinforce CanoHealth’s clinical model, workflows, and standard-of-care expectations
  • Work with the Territory Medical Director to develop and implement quality improvement projects, and provide education as it relates to quality of medical record documentation and HEDIS gaps.
  • Assist the Territory Medical Director in optimizing utilization of CanoHealth’s services and resources.
  • Participate in the implementation of policies and procedures as required by regulatory agencies that govern the practice of medicine in collaboration with the Territory Medical Director, CMO, Legal, and Human Resources Departments.
  • Ensure compliance with adverse incident reporting requirements and escalate to Operations, Territory Medical Director, and Risk Management.
  • Meet monthly with Health Center Managers and DOs to review operational and clinical opportunities and ensure proper communication across all departments within the organization.
  • Collaborate with district administrative staff as it relates to timely locking of patient medical records, and related EHR tasks to ensure patient safety.
  • Evaluate and address medical grievances from clinicians, and coordinate solutions in partnership with Territory Medical Director, VP of Operations, and escalates to CMO.
  • Ensure and maintain proper clinician staffing at all assigned medical centers.
  • Participate in clinical committees, where you will be responsible for gathering, analyzing, and presenting clinical, financial, quality, utilization, and performance data to support decision-making, policy development, and quality improvement initiatives.
  • Actively participate in conflict resolution.
