About The Position

Mountain View Hospital is looking for a Discharge Planning Assistant to join our team! The discharge planning assistant is responsible for providing accurate and timely administrative and technical support to the discharge planning and utilization management teams, helping ensure a positive patient experience, effective coordination of care, utilization review processes, and compliance with both hospital policy and CMS guidelines.

Requirements

• High school diploma or equivalent.
• Associate degree or 2 additional years of experience in lieu of a degree.
• Basic computer knowledge.
• Effective written and oral communication skills.
• Ability to comprehend and follow detailed instructions.
• Ability to present information and respond to questions from patients and staff.
• Ability to listen and seek clarification for accurate information dispersal.
• Ability to organize, recognize and correct inefficiencies.
• Ability to handle and prioritize multiple requests.

Responsibilities

• Provide administrative and technical support to discharge planning and utilization management teams.
• Ensure a positive patient experience.
• Coordinate care effectively.
• Assist in utilization review processes.
• Ensure compliance with hospital policy and CMS guidelines.
