About The Position

Discharge Planner Opportunity - Full Time - Days Cumberland Hall Hospital is a 97 bed, licensed, acute psychiatric hospital located in beautiful Hopkinsville, KY. Cumberland Hall features individual units for youth, adults and active-duty military, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average, over 1500 patients receive care from our compassionate health care team each year at Cumberland Hall Hospital. Requirements

• Bachelor's degree in Human Services or a related field (Psychology, Social Work, Sociology, Counseling, etc.) from an accredited college or university is required.
• Minimum of six (6) months of experience working in a mental health hospital setting is required.

Nice To Haves

• Prior experience in discharge planning is not necessary but may be beneficial.

Responsibilities

• Promptly engage with referral sources within 24 hours of patient admission to ensure seamless coordination of care.
• Design and oversee comprehensive discharge plans, including scheduling follow-up appointments and maintaining proactive communication with parents and key family members to support continuity of care.
• Provide tailored education to patients and families regarding discharge options, considering diagnosis, prognosis, available resources, and individual preferences for home care services.
• Maintain accurate and timely documentation of assessments, discharge plans, and related actions in the medical record, in alignment with departmental standards; prepare required reports and ensure proper recordkeeping.
• Act as a trusted resource and advisor to hospital staff, offering expertise on discharge planning protocols and applicable federal, state, and local regulations; evaluate and communicate the benefits, limitations, and implications of ongoing care as appropriate.
