About The Position

- The Discharge Planner at LIFESTREAM BEHAVIORAL CENTER is responsible for coordinating and facilitating the discharge process for patients transitioning from inpatient care to community-based services. This role ensures that patients receive appropriate follow-up care and support, aiming to enhance their recovery and integration into the community.

Requirements

  • Bachelor's degree in Social Work, Psychology, Nursing, or a related field.
  • Minimum of 2 years of experience in discharge planning or case management within a healthcare or behavioral health setting.
  • Experience working with diverse patient populations, including those with mental health and substance abuse issues.
  • Familiarity with community resources and services available for patient support post-discharge.
  • Strong understanding of discharge planning processes and best practices in behavioral health care.
  • Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare teams.
  • Ability to assess patient needs and develop comprehensive discharge plans tailored to individual circumstances.
  • Proficiency in electronic health records and documentation practices.
  • Problem-solving skills to address barriers to discharge and facilitate smooth transitions.
  • Knowledge of legal and ethical standards related to patient care and discharge planning.

Responsibilities

  • Collaborate with healthcare professionals, patients, and families to develop comprehensive discharge plans tailored to individual needs.
  • Assess patient requirements and resources to ensure a smooth transition from inpatient care to home or other facilities.
  • Coordinate with external agencies and community resources to arrange necessary services such as home healthcare, outpatient therapy, or social services.
  • Conduct discharge planning meetings and communicate effectively with all parties involved to ensure understanding and agreement on the discharge plan.
  • Monitor and review patient progress post-discharge to evaluate the effectiveness of the plan and make adjustments as necessary.
  • Maintain accurate and up-to-date documentation of discharge plans, patient interactions, and follow-up activities.
  • Educate patients and families about available resources and support systems to facilitate continued recovery and prevent readmission.
  • Work collaboratively with the healthcare team to identify potential barriers to discharge and develop strategies to overcome them.
