About The Position

This position is responsible for providing ongoing and systematic utilization review and discharge planning efforts (in coordination with the treatment team) on all patients assigned by Director; coordinating patient care; advocating for the patient and facility; and promoting quality of care. Requirements

• Requires organizational, verbal, interpersonal and customer service skills, grammar and spelling, the ability to read/comprehend written instructions and follow verbal instructions.
• Ability to establish and maintain effective working relationships with employees, managers, referral sources, patients/families, community resources, etc.
• Ability to exercise professional independent judgment in planning own work.
• Must be flexible with work hours to accommodate work volumes.
• Knowledge of utilization management and managed care environment is helpful.
• Bachelors degree in social work, psychology, counseling, or related field.
• Requires a minimum of six (6) months' experience in a health care setting.
• Must meet or exceed hospital and regulatory agency training requirements.

Nice To Haves

• Experience in case management preferred.
