Website: https://www.vallevistahospital.com/ Position Summary: FT Days This employee is responsible for the discharge planning for patients receiving care on the acute mental health units. The Discharge Planner provides patients with a review of treatment options beyond VVHS. The Discharge Planner represents VVHS in coordination of treatment follow up for patients within the community. The Discharge Planner works closely with other members of the social services team from the time of intake to plan for an appropriate discharge plan. Requirements

Education: Bachelor's Degree in Social Work or other social science is required.
Experience: Six months to one year of experience working with a psychiatric in-patient population preferred.
Additional Requirements: Must be at least 21 years of age.
Work Hours: 8:30 am - 5:00 pm Monday - Friday. Must be able to work periodic weekends

Responsibilities
