About The Position

The Discharge Planner works under the director of the RN Case Manager and/or /Social Worker to complete referrals for post-acute services for patients and assists with tasks that do not require a clinical license or degree.

Requirements

• MINIMUM EDUCATION: High School diploma or equivalent
• MINIMUM EXPERIENCE: 6 months in health care environment.
• Possesses excellent organizational, verbal/written communication skills, problems solving and computer literacy skills.
• Data Analytics skills.
• Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire
• Non-Violent Crisis Intervention (NVCI) within 60 days of hire.

Nice To Haves

• PREFERRED EDUCATION: Associate’s or Bachelor’s Degree from an accredited college
• PREFERRED EXPERIENCE: Acute hospital experience
• PREFERRED CERTIFICATIONS/LICENSURE: Paramedic, EMT or Nursing Assistant certifications.

Responsibilities

• Arranges post-discharge services
• Creates and follows-up on electronic referrals using the Tenet Case Management system
• Reviews patient choice letters with patients/families for required signatures
• Provides follow up Important Message to Medicare patients prior to discharge
• Communicates with patient, families and other members of the care team
• Completes tasks assigned by Case Manager and Social Work staff
• Make copies, send faxes, and completes phone calls
• Completes process reviews or audits as requested
• Completes established competencies for the position within designated introductory period
• Other related duties as assigned
