About The Position

As an integral member of a multidisciplinary team, the Inpatient Discharge Planner works collaboratively with the Acute Treatment Unit (ATU) team to oversee, organize and/or coordinate discharge planning. Duties include but are not limited to: meeting with clients on a daily basis; conducting safety planning and ensuring it is occurring and/or reviewed every 24hrs; reviewing, verifying, and tracking the completion of all paperwork in accordance with compliance regulations, including legal paperwork; scheduling follow-up appointments; and coordinating discharge communications and transportation arrangements. Supports and/or conducts Utilization Management (UM) as needed. Maintains working knowledge of Diversus Health programs and external community resources. Participates in client de-escalation as needed.

Requirements

  • Bachelor’s Degree in a behavioral health or healthcare related field required.
  • Two (2) years of professional work experience or volunteer experience in a behavioral health field; acute care setting strongly preferred.
  • Desire to work with people who have mental illness.
  • A thorough understanding of systems of care and integrated care (physical and behavioral health).
  • Excellent organizational and team leadership skills.
  • Ability to take initiative and use critical thinking to independently problem solve.
  • Excellent verbal and written communication skills. Possesses the ability to accurately communicate, implement and document the services provided to client in addition to easily building rapport with clients and other team members.
  • Proficient computer skills, to include Microsoft Suite, with the ability to ty pe 25+ w.p.m. for the input and output of client information, using electronic medical records.

Responsibilities

  • Collaborates with the ATU team to oversee, organize, and/or coordinate discharge planning. Duties include but are not limited to: meeting with clients on a daily basis; conducting safety planning and ensuring it is occurring and/or reviewed every 24hrs; reviewing and verifying the completion of all paperwork in accordance with compliance regulations, to include legal paperwork; scheduling follo w-up appointments; and coordinating discharge communications and transportation arrangements.
  • Understands Utilization Management (UM) and insurance verification process. Supports and/or conducts UM as needed.
  • Maintains working knowledge of Diversus Health programs and external community resources.
  • Conducts relevant team-based or evidence-based skills groups. Accurately documents participation in Electronic Medical Records (EMR).
  • Assists with the monitoring of clinical and environmental risk issues, while consistently following all safety procedures. Duties include but are not limited to completing Q15 watches, line of sight checks, and other safety checks; monitoring of utensils, pencils, and contraband on the unit; and conducting one to one’s per provider orders.
  • Supports nursing staff in the client admissions process as needed. Duties include but are not limited to storage of client’s personal property and associated documentation, and orienting clients to the unit.
  • Interacts with clients on the unit while maintaining healthy therapeutic boundaries. Participates in the de-escalation of clients in crisis when appropriate.
  • Assists in the supervision of the ATU visitation process. Assures appropriate documentation/consents have been completed. Reports any issues or concerns to the charge nurse on duty.
  • Helps providers with daily client care and discharge duties within scope of role, to include but not limited to patient education, outreach, crisis response planning, coordination of continued care and transportation, processing the return of personal items, and follow-up telephone calls within 48 hours of discharge.
  • Aids in the monitoring of patient nutritional intake. Accurately documents intake in EMR.
  • Assists in maintaining a safe and secure environment for staff and clients. Uses trained skills of Handle with Care in safely managing crisis situations with escalated clients and responds according to approved protocols for crisis situations and restraint/seclusion events.
  • Actively engages in team meetings, in-service trainings, and supervisory sessions as required. Completes compliance and other applicable trainings within required time frame. Performs other various shift duties as assigned by management.
