As a member of the multi-disciplinary treatment team, the Discharge Planner provides duties including: completion of Social Assessments, aftercare appointments, and referral communication. The Discharge Planner interfaces with referral sources, community service agencies, and businesses. The Discharge Planner ensures compliance for quality patient care and compliance with Joint Commission standards, State Health Department requirements and all other regulatory standards. Requirements

  • Current CPR and Handle with Care certifications, offered onsite.
  • Bachelor’s Degree in Social Work, Psychology, Counseling, or related field.
  • Must have documented experience working with children and adolescents.
  • Must have effective verbal and written communication skills to communicate in a clear, concise manner with referral sources, hospital staff and the public, as well as provide accurate and timely documentation and record keeping.
  • Good organizational and time management skills to cope with a fast-paced environment and frequent interruptions, complex schedules and fluctuations in census is required.
