Website: https://midwest-center.com/ Position Summary: The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. Requirements

  • Master's degree from an accredited school in Social Work or a clinical-related mental health field or ability to obtain within one year of hire
  • Minimum of 3 years experience in a psychiatric health care facility with direct experience in discharge planning required.
  • Valid driver's license and safe driving record required.
  • Must be at least 21 years old.

Responsibilities

  • Communicate with referral sources within the first 24 hours of admission
  • Identify all outside professionals currently working with the patient so appropriate discharge plans can be completed, scheduled with parents or significant family contacts
  • Identify needs of referral sources and/or professional during patient's hospitalization period
  • Oversee continued communication of treatment progress and discharge plans to outside interest parties and transfer requireed documents to outside agents
  • Work with the Business Development Department on customer satisfaction
  • Assist with chart audits & completion of medical records
  • Inform patients and families of discharge planning options
  • Act as an educational resource to hospital personnel - benefits, limitations
  • prepare reports & attend meetings as necessary
