About The Position

As a Discharge Planner, you will play a vital role in bridging the gap between hospital care and post-discharge needs. You will work directly with patients, families, and the interdisciplinary team to ensure a smooth transition to outpatient services or other support systems.

Requirements

• Bachelors degree in related field required
• Strong written and verbal communication skills with the ability to adapt to diverse audiences.
• Knowledge of community resources and services for patients and families.
• Ability to work collaboratively in a multidisciplinary team environment.

Responsibilities

• Provide pre- and post-discharge information, assistance, and referrals to patients and families.
• Collaborate with the interdisciplinary team to coordinate discharge activities.
• Facilitate adult, child, and adolescent group therapy sessions.
• Assess patients' needs, including housing, transportation, and continuing care, and make appropriate referrals.
• Notify outpatient providers of hospitalization with patient consent.
• Maintain detailed and timely documentation of discharge plans and related processes.
• Develop and evaluate educational materials for patients and families.
• Participate in clinical supervision sessions and team meetings.
• Support Utilization Review efforts to optimize patient care and insurance benefits.
