About The Position

POSITION SUMMARY: The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Assist Licensed Therapists in developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care.

Requirements

• Bachelors in SW or related field
• Minimum of one (1) year experience (preferred) in a psychiatric health care facility with direct experience in discharge planning and/or case management.

Nice To Haves

• Masters in Social work or related field
