Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties. Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care.
Requirements
- Bachelor's degree in related field required, or associate degree in related field with QHP required.
- Experience in a healthcare/medical setting, preferred.
- Strong writing and communication skills are required.

- Experience working with a diverse population and a strong understanding of multicultural issues is preferred.
