About The Position

Wilmington Treatment Center is North Carolina's premier substance abuse recovery hospital and rehabilitation center located in the beachside community of Wilmington, NC. Our facility has grown from a 22 bed hospital in 1984 to a 260 bed hospital spanning 3 campuses. Requirements

• Associates degree in nursing or human services field required.
• Bachelor's degree in social work or human services field preferred.
• One or more years' experience in a clinical setting preferred.
• One or more years' experience in discharge planning preferred.
• Crisis Prevention and CPR certifications (offered on site).
• Ability to work with male and female adult (18+) patients with chemical dependency and mental health disorders.
• Teamwork and excellent customer service skills.

Responsibilities

• Develop a continuing plan based on patient's needs that promotes their long-term health and relapse prevention.
• Set discharge appointments, including group or individual therapy, psychologist or psychiatrist.
• Make arrangements for community resources to facilitate discharge.
• Provide all needed documentation to other providers to facilitate the continuum of care.
• Ensure all documentation regarding patient discharge planning is clear, concise, and timely.
