As a full-time Discharge Planner at the Hampton-Newport News Community Services Board, you will play a pivotal role in facilitating patient discharges from State Hospitals, ensuring smooth transitions for clients returning to the community. You'll consult with staff from Eastern State Hospital, Central State Hospital, or local psychiatric facilities to coordinate comprehensive discharge and after-care planning. The position entails conducting mental status evaluations and client assessments to make informed treatment recommendations and develop tailored service plans. Acting as a liaison among various facilities, you will advocate for clients, monitor their behavior, and provide crisis intervention as necessary. Additionally, you'll participate in the allocation of consumer funding and represent the agency on the Regional Discharge Assistance Committee (REDAC). Your involvement will extend to training program staff, collaborating with law enforcement and health providers, and implementing policy changes to enhance service delivery. This dynamic role is essential in promoting a seamless support system for clients in need.
