BayCare Health Systems is currently seeking a Discharge Planner Pool to join our outstanding and compassionate team at Mease Dunedin!

Facility:  Mease Dunedin
Location: Dunedin, FL
Status: Pool, Hourly
Shift Hours: 8:00 am-4:30 am
Weekend Work: 4 Weekend Days/ Month
On Call: No

Requirements

• BLS (Basic Life Support)
• Behavioral Health - 1 year
• Child and Adolescent - 2 years (SIPP Only)
• Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing

Responsibilities

• Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
• Possesses a clear understanding of discharge and transition planning and linkage with community resources.
• Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
• Provides support as active member of the treatment team.
• Will perform other duties as assigned by supervisor.
• Travel may be required.
