About The Position

The Discharge Planner - MSW position is located in Elmhurst, IL, and is a full-time role requiring 40 hours per week. The working hours are Monday to Friday from 8 am to 4:30 pm, with rotating weekends and holidays every 5-6 weeks. The role may require travel to Edward Hospital in Naperville, IL for coverage as needed. The Discharge Planner will provide comprehensive discharge planning services, assess patients' emotional and psychosocial needs, and coordinate necessary post-acute care and discharge activities.

Requirements

  • Master’s Degree in Social Work.
  • Minimum one year medical social work experience in a healthcare setting or a clinical rotation in an acute care hospital setting.
  • Current State of Illinois Social Worker (LSW or LCSW) licensure.

  • Minimum one year previous hospital social work experience.

Responsibilities

  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served.
  • Provides comprehensive discharge planning services for identified and referred cases.
  • Assesses patients' emotional status, psychosocial stability, adjustment to injury, illness or disability, and family support system.
  • Conducts post-acute care and discharge planning activities, including home care referrals and durable medical equipment.
  • Continuously communicates discharge plans to patients, families, and staff.
  • Screens assigned unit cases based on established criteria and actively pursues identified needs.
  • Provides psychosocial therapies to assist patients and families in coping with illness and disability.
  • Attends patient-centered conferences and complex care rounds to discuss key factors affecting patients' treatment responses.
  • Educates patients and families regarding the effects of illness and disability.
  • Maintains accurate and timely medical records.
  • Participates in required meetings including interdisciplinary treatment planning and in-service training.
  • Remains informed about agencies and resources to assist patients and families post-discharge.
