About The Position

The Discharge Planner - MSW position is a per diem role located in Elmhurst, IL, requiring coverage for weekends and on an as-needed basis. The role involves providing comprehensive discharge planning services for patients, assessing their emotional status, psychosocial stability, and family support systems. The Discharge Planner will conduct post-acute care activities, coordinate home care referrals, and maintain communication with patients and families throughout the discharge process. Requirements

  • Master’s Degree in Social Work.
  • Minimum one year medical social work experience in a healthcare setting or clinical rotation in an acute care hospital setting.
  • Current State of Illinois Social Worker (LSW or LCSW) licensure.

Responsibilities

  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served.
  • Provides comprehensive discharge planning services for identified and referred cases.
  • Assesses patients' emotional status, psychosocial stability, adjustment to injury, illness or disability, and family support system.
  • Conducts post-acute care and discharge planning activities for referred patients.
  • Continuously communicates discharge plans to patients, families, and staff.
  • Screens assigned unit cases based on established criteria and pursues identified needs.
  • Provides psychosocial therapies to assist patients and families in coping with illness and disability.
  • Attends patient-centered conferences and complex care rounds to discuss key factors affecting patients.
  • Educates patients and families regarding the effects of illness and disability.
  • Maintains accurate and timely medical records.
  • Participates in required meetings including interdisciplinary treatment planning and in-service training.
  • Remains informed about agencies and resources for continuity of care after discharge.
