As a Discharge Planner, the primary responsibility involves coordinating and executing discharge planning services, identifying local resources, and arranging appointments with programs or providers to ensure a smooth transition for individuals. Additionally, Discharge Planners may offer individual and/or group psychoeducational sessions for individuals scheduled for release into the community, fostering their preparedness and support network.
