• Monitors clinical information to support psychosocial assessments and treatment continuity among healthcare providers.
• Conducts needs assessments considering social determinants of health.
• Plans and implements care in collaboration with the multi-disciplinary team.
• Coordinates activities with patients to meet their goals.
• Collaborates with the interdisciplinary team to adjust patient care levels and services as needed.
• Documents all interventions and services provided.
