About The Position

The Discharge Planner - Case Management position is a full-time role located at Corona Regional Medical Center in Corona, CA. Reporting to the Case Management Director/Manager, this position provides evaluation and psychoeducation to patients and their families or significant others, assisting them in coping with psychosocial problems related to acute and chronic illness or injury. The role involves assessing the patient's current living environment and general medical/social needs, providing interventions to maximize the patient's reintegration into the home and community, and delivering competent care for patients throughout their lifespan. The discharge planning process is integrated into the responsibilities of this position.

Requirements

• Certificate as Social Service Designee or one year's experience as a Social Work Assistant or discharge Planner in lieu of Certificate.
• Bachelor's degree in psychology or sociology with equivalent job-related experience.
• Minimum of 12 months clinical experience in the field of Mental Health.
• Age-specific education and/or experience working with adult and geriatric behavioral health.
• Excellent written and verbal communication skills.
• Current BLS (CPR) Certificate.
• Knowledge of Patient.
• Experience with discharge planning and case management.

Responsibilities
