About The Position

The Discharge Nurse Navigator (DNN) as part of the Discharge Navigator Nurse team, utilizes evidence-based approaches to the hospital discharge process, assisting patients/families navigating their hospitalization course to assure a safe transition of care to home. The DNN conducts daily rounding on inpatients, proactively identifying potential barriers/gaps to discharge, collaborating and following up with the appropriate members of the healthcare team, and assist in optimizing each patients’ discharge planning. The DNN reinforces education of diagnosis, plan of care, medications, home care needs, etc., utilizing teach-back technique in all aspects of care from admission throughout their admission to time of discharge. As part of the DNN team, the DNN actively participates in quality improvement activities to improve processes and outcomes as related to assuring a safe transition home for CHOC’s patients and families.

Requirements

  • Minimum two (2) years of inpatient pediatric experience.
  • Bachelor of Science degree in Nursing from an accredited school.
  • Knowledge of and ability to provide appropriate family-centered patient care based upon age appropriateness and cultural and spiritual needs.
  • Understanding of pediatric acute care inpatient hospital processes and workflow.
  • Excellent customer satisfaction skills, understand and exceed customer expectations.
  • Effective communication skills.
  • Ability to prioritize and complete duties in a timely and organized manner; work collaboratively and in a positive manner with others; prepare timely and accurate documentation and reports; perform work according to CHOC policies and procedures, professional standards of care, and applicable laws.
  • Demonstrate flexibility and creativity in response to unexpected changes in workload/staffing.

  • CA Registered Nurse (RN)
  • Basic Life Support (BLS)
  • Pediatric Advanced Life Support (PALS).
  • Neonatal Resuscitation Provider - Essentials (NRP) - American Heart Association.
  • Neonatal Resuscitation Provider - Advanced (NRP) - American Academy of Pediatrics.

Nice To Haves

  • Five (5) + years of nursing experience, two (2) + years of inpatient pediatric experience.
  • Master of Nursing degree.
  • Nursing certification.
  • Bilingual English/Spanish.
