About The Position

Facilitates the transition of a patient and family from one environment to another by providing them with a comprehensive continuous plan of care; e.g., arranging transportation, preparing information to facilitate patient's movement to the most appropriate level of care.

Requirements

• BSW, Bachelor's degree, or LPN required
• Screening and assessment skills and expertise in care coordination and/or utilization management in a health care setting required
• Five or more years of related experience may be substituted for the credentials
• Active and current appropriate licensure through the Department of Professional Regulation, State of Florida as indicated
