About The Position

Gateway Rehab Center (GRC) is excited to offer a rewarding opportunity for a Discharge Coordinator at our Center Township, PA location! In this vital role, you'll be the bridge between our patients and their next steps—working closely with therapists and patients to create smooth, supportive discharge plans. As Coordinator, you'll facilitate referrals and aftercare planning for patients in our 3.5 level of care, including extended care, halfway and recovery houses, outpatient programs, medical and psychiatric appointments, and MOUD/MAUD services. Requirements

• High School Diploma
• Understanding of 12 step recovery programs and the recovery process preferred.

Responsibilities

• Coordinates discharge planning with therapist input and treatment planning.
• Assists patients in scheduling medical, legal, drug and alcohol, MOUD/MAUD and psychiatric appointments.
• Works with patients to coordinate extended care or recovery housing.
• Complete all documentation in patients' charts regarding internal transfers and scheduling.
• Provides comprehensive written and verbal follow up with referral source during treatment and at discharge.
• Networks with referral sources and outside treatment providers and other agencies.
• Coordinates and communicates with other Gateway programs and departments.
• Provides direct patient care as needed, including crisis intervention.
• Facilitate patient surveys upon discharge.
• Any other duties assigned by supervisor.
• Attends Gateway mandatory trainings and in-services.
