About The Position
The Discharge Assistant supports the Care Coordination and Social Work staff to aide in safe and efficient transitions of care. They are responsible for coordination of information andmunication and other designated responsibilities for the care management team.
Requirements

- A minimum of one-year experience in healthcare setting preferred.
