About The Position

The People Concern has partnered with the Department of Health Services (DHS) and the Office of Diversion and Re-entry to provide housing and Intensive Case Management Services (ICMS) to persons being released from jail with a prior history of homelessness. Participants are linked to the ODR program while they are in jail. Once released, Case Management staff will assists individuals matched to an ODR Project Based Housing site with the lease up process. Once housed, the goal is to help individuals with housing stability and retention by providing intensive services. Director, Project Based Housing, Jail Programs, will provide administrative oversite for all project based DHS Jail Programs sites, including partnering and collaborating with the Department of Health Services (DHS) Office of Division and Re-entry (ODR). The Director will oversee the managerial/administrative operations and serves as the Liaison with ODR program staff on program operations, reporting, outcomes, etc

Requirements

  • Mental Health Provider holding an Masters of social work (MSW), or an equivalent graduate degree in a related mental health field.
  • Extensive experience working with individuals with severe and persistent mental illness and co-occurring disorders as well as experience working with the criminal justice population.
  • Able to work in a high tolerance Harm Reduction model with Members who have multiple barriers
  • Extensive knowledge of local and federal government benefits and entitlements

Nice To Haves

  • At least two (2) years of management/supervisory experience preferred
  • Licensed Mental Health Provider holding an MSW or equivalent, with at least two (2) years of post‑licensure clinical experience and eligibility to supervise individuals toward licensure.

Responsibilities

  • Work with staff to develop effective, trusting relationships with clients using a client-centered approach that includes motivational interviewing, trauma-informed care and harm reduction.
  • Provide administrative oversite and systems organization with both of the DHS funded jail teams, including interacting with DHS Jail linkage staff and attending all case conference with DHS designated staff.
  • Create procedures related to ODR to ensure continuity of care and effective client care.
  • Provide clinical consultation to staff working with difficult clinical situations, and suggest appropriate clinical interventions.
  • The supervision and coordination with the Senior Direct of DHS Jail Program, provide daily coordination and oversight of program operations and program activities, including supervision of Program Managers, Licensed Clinical Supervisors, and contracted psychiatrists.
  • Provide supervision to staff working towards licensure.
  • Work with Brilliant Corners, and DHS to address policy or client related advocacy issues.
  • Assist Program Management staff in recruitment, selection, training, supervision, and evaluation of direct service staff.
  • In partnership with other Directors within the Housings Department, provide leadership to department wide assistant directors, program managers and case managers.
  • Ensure that data is being entered into the DHS database (CHAMP) in a timely manner.
  • Monitor that staff are providing appropriate level of service and that progress notes are being completed in a timely manner.
  • Organize documentation for housing applications (HACoLA, HACLA and Brilliant Corners)
  • Ensure intake and psychosocial assessment are completed for all referrals within timeline listed in DHS ICMS work order.
  • Coordinate psychiatry services and organize invoice documents to include in monthly billing
  • Manage all ODR administrative needs (tracking and managing court dates and court progress reports, tracking ODR releases, coordinating interim referrals, requesting medications upon release, and coordination of all releases to ensure immediate responsiveness.
  • Attend ODR team case conference and act as the clinical consultant.
  • Ensure data entry accuracy for reporting requirements’ outcomes.
  • Oversee chart accuracy for audit purposes.
  • Participate in staff meetings, trainings and retreats.
  • Maintain client confidentiality at all times and ensure to follow agency, state and HIPAA laws.
  • Provide crisis intervention for clients when necessary.
  • Follow up on client grievances in a timely manner
  • Ensure that all Annual Reviews are completed in a timely manner.
  • Provide oversite regarding hiring and ensure Diverse and Equitable hiring practices to appropriately reflect the diversity of the population served.
  • Ensure adherence to policies and procedures
  • Become LPS designated and write applications for 5150 hold.
  • Other duties as assigned
