About The Position

The Director of Wound Care Services provides strategic, clinical, and operational leadership for the organization’s wound care program across clinic-based and mobile care settings. This role ensures the delivery of high-quality, evidence-based wound care to patients across the post-acute care continuum while supporting organizational growth, regulatory compliance, interdisciplinary collaboration, and continuous quality improvement. The Director serves as the clinical expert and program leader for wound prevention, treatment, and education, working closely with physicians, advanced practice providers, nurses, therapists, and operational leadership. Requirements

  • Minimum 5–7 years of clinical wound care experience
  • Minimum 3–5 years of leadership or management experience, preferably in home health, hospice, outpatient, or ambulatory care settings
  • Advanced knowledge of wound assessment, treatment modalities, and prevention strategies
  • Strong leadership, communication, and interpersonal skills
  • Demonstrated ability to lead multidisciplinary teams
  • Excellent organizational, analytical, and problem-solving skills
  • Proficiency with electronic medical records and data reporting tools
  • Ability to travel to clinic sites and patient care locations as needed

Nice To Haves

  • Experience providing wound care in both clinic and mobile/home-based environments strongly preferred

Responsibilities

  • Provide leadership and oversight for all wound care services delivered in clinic and mobile (home, facility, hospice) settings
  • Establish and maintain evidence-based wound care protocols, clinical pathways, and treatment standards
  • Support appropriate product selection, utilization, and formulary management
  • Develop, implement, and evaluate wound care programs aligned with organizational goals and community needs
  • Collaborate with executive leadership on service line growth, program expansion, and resource planning
  • Oversee scheduling, staffing, and workflow optimization for clinic and mobile wound care services
  • Monitor program performance metrics, outcomes, and patient satisfaction
  • Ensure compliance with CMS, Medicare Conditions of Participation, state regulations, accrediting bodies, and organizational policies
  • Lead quality improvement initiatives related to wound healing outcomes, infection prevention, and documentation accuracy
  • Review adverse events, incidents, and wound-related complications; implement corrective action plans as needed
  • Collaborate with physicians, nurse practitioners, physician assistants, therapists, and nursing leadership to ensure coordinated patient care
  • Serve as a clinical liaison between home health, hospice, clinics, referral sources, and community partners
  • Participate in care conferences and provide consultation on complex cases
  • Develop and deliver wound care education, competency programs, and ongoing training for clinical staff
  • Mentor and support wound care clinicians, fostering clinical excellence and professional growth
  • Stay current with emerging wound care technologies, products, and best practices
  • Ensure accurate, timely, and compliant clinical documentation across all care settings
  • Utilize data and reporting to evaluate outcomes, identify trends, and drive performance improvement
  • Support proper coding, billing, and documentation practices related to wound care services
