About The Position

We are seeking a Director of Wellness to join our growing team. This individual is responsible and accountable for managing nursing care to Independent, Assisted Living, and Memory Care residents. Oversees the delivery of services and staffing for all levels of care; coordinates with outside physicians and health professionals to ensure appropriate care for residents. What You Will Do: Responsible for the ongoing assessment of physical, mental, and emotional needs of independent, assisted living, and memory care residents. Provides supervision of care and services provided to residents. Interviews, hires, supervises, trains, motivates, and evaluates performance of staff. Develops short- and long-term goals and objectives in consultation with community management and consistent with Sequoia Living Mission, Vision, Core Values, and Commitment to Inclusion. Fosters an environment of employee engagement and resident satisfaction. Ensures personal assistance with daily living and personal care in resident’s independent living, assisted iving, and memory care apartments in accordance with an individual's goals and/or service plan. Accountable for supporting the attainment of performance goals as measured by specific indicators in Financial, Marketing, Productivity, Risk Prevention, and Resident Satisfaction. Responds to emergency situations and expedites indicated treatment; secures follow-up assistance, as appropriate. Provides assistance with scheduling visits to outside physicians; communicates with and assists outside physicians to ensure that changes in treatment are processed and residents are assessed and evaluated appropriately for medical, emotional, and nursing needs. Oversees the administration of medications and treatments according to physicians’ orders. Consults family members when indicated to ensure that residents’ needs are met. Informs Executive Director of any events which may necessitate intervention. Coordinates the functions and processes of the clinic, assisted living, and memory care environments including systems, space utilization, supply and equipment availability; and manages cost containment issues. Responsible for maintaining residents’ medical charts. Assesses the need for additional help or services for residents; coordinates with residents, their responsible party, and/or family members, with other departments, and with outside health providers, as necessary, to meet needs. Assures that care by outside home care and home health professionals retained by residents satisfy Sequoia Living criteria. Monitors the service plans of residents utilizing home care and home health services. Participates in the evaluation of the need for permanent or transient stays at all levels of care including Viamonte’s assisted living and memory care and outside hospitalization and skilled nursing. Assists in the coordination of health-related discharges and admissions of Viamonte residents; tracks residents admitted to outside acute care or skilled nursing; communicates information with the Executive Director. Coordinates monthly Resident Review with the care team. Responsible for maintaining service plans and all other DSS required forms. Responsible for Independent Living, Assisted Living, and Memory Care Continuous Quality Improvement activities by ensuring nursing domains are monitored and reports are prepared. Sensitivity and understanding of issues related to aging. Patience and tact in dealing with residents and families/responsible parties. Thorough knowledge of nursing theory and practice. Knowledge and skill to make decisions regarding unit activities. Physical skills and ability to perform work that requires continual standing, walking, stooping, bending and lifting of at least 50 pounds. Graduated from an accredited school of nursing and licensure to practice as an RN in the State of California. B.S.N. degree preferred. Experience equivalent to a minimum 2-5 years nursing experience which includes supervisory responsibility in a geriatric setting. Basic Life Support certificate. Dementia experience and RCFE training preferred. Supervises up to 20 staff in the Clinic, Assisted Living, and Memory Care units.
