About The Position

The Director of Telehealth is responsible for planning, implementing, and managing telehealth programs within the home health organization. This role oversees remote patient monitoring, virtual care services, telehealth technology integration, clinical workflow development, and regulatory compliance to improve patient outcomes, expand access to care, and ultimately reduce rehospitalizations.

Requirements

  • Bachelor's degree in Nursing, Healthcare Administration, Public Health, Business, or related field required.
  • 5+ years of leadership experience in healthcare (required)
  • Experience implementing telehealth or remote patient monitoring programs (required)
  • Knowledge of CMS and Medicare home health regulations

Responsibilities

  • Develop and execute the organization's telehealth strategy for home health services.
  • Oversee the implementation and expansion of remote patient monitoring (RPM) and virtual care programs.
  • Utilize evidence-based practices to ensure the highest level of clinical recommendations are delivered to Adaptive’s patients.
  • Collaborate with executive leadership to align telehealth initiatives with organizational goals. (e.g. integrations, acquisitions, payer strategy)
  • Evaluate new telehealth technologies and recommend adoption where appropriate.
  • Integrate telehealth services into home health clinical workflows, ensuring all data ends up in the EMR.
  • Work with nursing, therapy, and care coordination teams to ensure effective use of telehealth tools. (e.g. patients that need telehealth services receive said services).
  • Develop protocols for: Remote patient monitoring Virtual visits Patient triage and escalation Early detection, intervention, and prevention of hospitalizations
  • Monitor program outcomes and optimize service delivery.
  • Manage telehealth platforms, devices, and digital health systems.
  • Collaborate with IT teams to ensure secure and reliable technology infrastructure.
  • Evaluate and manage relationships with telehealth vendors and technology partners.
  • Ensure integration with EHR/EMR systems either by direct integration or transcription.
  • Ensure telehealth programs comply with: Medicare and Medicaid regulations HIPAA privacy and security standards State telehealth regulations (if applicable)
  • Maintain EMR documentation and reporting for telehealth services.
  • Track and analyze key performance metrics including: Hospital readmission rates Patient engagement Telehealth utilization
  • Use analytics to improve program performance and patient care.
  • Hire and train telehealth staff on telehealth tools and workflows.
  • Develop telehealth training and competency programs.
  • Ensure best practices in remote care delivery.
  • Develop or implement existing patient education materials for telehealth use.
  • Address patient access barriers such as technology literacy to ensure all patients can access the benefit.
