About The Position

Recruit Secure is collaborating with a beautiful Skilled Nursing Facility (SNF) located in Oceanside, NY that is seeking a dynamic and energetic individual to take on the full-time role of Director o f Social Services. The SNF is a long-term nursing and rehabilitation facility with about 100- beds and is dedicated to providing the highest levels of care and commitment to its residents, their families, and their employees. Requirements

Master’s degree in Social Work, LMSW preferred b ut not mandatory
Must have at least 3 years of experience working as a Licensed Social Worker within a Nursing or Rehabilitation Facility.
At least 1 year of experience in a Senior role
Should have a good working knowledge of available social services and other community resources
Must have excellent communication

Responsibilities

Develops the Social Services plan of care for residents, identifying goals and approaches and participating in inter-department care planning meetings.
Participates in Quality processes of the SNF Counseling residents and family members; facilitating family meetings
Complete Admissions, Psychosocial Assessments, and Advanced Directives and assures placement in medical records.
Oversees referrals for health services and facilitates discharge planning process for the department.
Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates and revisions to policies when and if needed.
Completing all required documentation for medical records; producing monthly departmental reports for the SNF Administrator.
