In this leadership role, you’ll help shape a supportive, person-centered environment for our residents and families, while working alongside a committed interdisciplinary team. What You'll Do: Lead and manage all aspects of the Social Services Department. Provide support for residents and families including assessments, discharge planning, and care coordination. Supervise and mentor staff, promote professional growth, and ensure regulatory compliance. Serve as a key liaison between residents, families, staff, and external care partners. Uphold excellence in service and communication, helping create a community of dignity and respect.
