Looking for a qualified Director of Social Services to join our team! We are searching for a Director of Social Services to lead our social services department and ensure residents, and their families receive the support they need. We recognize the vital role that social services plays in improving the well-being and quality of life for those in our care and are looking for a team builder who is excited about the opportunity to assist in building a facility culture. The primary purpose of the role is to be responsible for providing services to respond to the emotional needs of the residents and their families. You will also assist with resident admissions and the referral process.

  • Stay up to date with federal and state regulations and make policy recommendations to the Licensed Nursing Home Administrator.
  • Implement policies and procedures to identify and address the social and emotional needs of residents.
  • Participate in community planning to support facility services and resident needs.
  • Oversee discharge planning, development of social care plans, and participation in resident assessments.
  • Provide consultation to staff and community agencies to develop effective social service programs.
  • Develop and implement corrective action plans for social service deficiencies noted during survey inspections.
  • Maintain an up-to-date reference library of laws, standards of practice, and other essential materials.
  • Conduct and document social services evaluations, identifying resident problems and needs.
  • Maintain ongoing communication with case workers and external service providers.
  • Provide social work consultation to residents, families, and staff as needed.
  • Drive innovation and process improvements to enhance social services and resident care outcomes.
  • Bachelor's or Master's degree in Social Work, Human Services, or a related field (preferred).
  • Experience in long-term care, healthcare, or social services within a skilled nursing or similar setting.
  • Strong understanding of state and federal regulations governing social services in nursing facilities.
  • Excellent interpersonal, communication, and problem-solving skills.
  • Ability to build and maintain relationships with residents, families, and external agencies.
