About The Position
Our skilled nursing facility is seeking a Director of Social Services to fill a full-time position at our skilled nursing facility. The Director of Social Services will manage the social work, admissions, and discharge planning functions. This is a direct hire position; we are not an agency. Requirements
- Master's degree in a social services or related field required.
- Social work license is required - LICSW, LCSW or LSW
- Experience in social work, admissions, and/or discharge planning in a nursing home or other long-term care setting is required.
- Knowledge of the Medicaid process is required.
Responsibilities
- Reviewing admissions documents for appropriateness of placement in our facility
- Completing social services assessments, including but not limited to BIMS, PHQ-9, trauma screening
- Planning aftercare for residents discharging from the facility, including but not limited to housing, home care services, medical equipment, and follow up medical appointments
- Maintaining open lines of communication with residents and families regarding concerns
- Coordinating care plan meetings
- Maintaining advanced directives
- Completing documentation of relevant tasks in PointClickCare
