About The Position

We are seeking a Director of Rehabilitation who embodies this philosophy—someone who leads with clinical excellence, fosters a positive team culture, and is committed to helping each resident reach their highest level of functioning. In this key leadership role, the Director of Rehabilitation oversees all therapy services within the facility, guiding a multidisciplinary team of OTs, OTAs, PTs, PTAs, and SLPs. The Director ensures compliance with regulatory and payer requirements, supports quality outcomes, manages daily operations, and provides direct patient care when needed. This position plays a vital role in shaping both team performance and the resident experience.

Requirements

  • Licensed PT or OT (Bachelor’s, Master’s, or Doctorate).
  • 3–5 years of experience in skilled nursing, inpatient rehab, or post-acute care.
  • Prior supervisory or management experience preferred.
  • Strong understanding of PDPM, regulatory requirements, and documentation compliance.
  • Proven ability to lead, mentor, and inspire a multidisciplinary team.
  • Excellent communication, problem-solving, and relationship-building skills.
  • Commitment to ethical practice, resident-centered care, and continuous improvement.

Responsibilities

  • Lead and manage the rehabilitation department, ensuring consistent delivery of high-quality, patient-centered care.
  • Foster a collaborative, supportive, and engaged team environment rooted in Country Meadows’ values.
  • Oversee compliance with state/federal regulations, PDPM, documentation standards, and clinical best practices.
  • Monitor clinical outcomes and partner with therapists to drive quality improvement.
  • Support hiring, onboarding, coaching, mentoring, and performance evaluations for therapy staff.
  • Manage daily operations including scheduling, equipment readiness, workflow, and utilization.
  • Collaborate with the interdisciplinary team on care plans, progress reviews, and discharge planning.
  • Maintain accurate, timely departmental documentation and required reporting.
