About The Position

Director of Rehabilitation CommuniCare Rehab , a member of the CommuniCare Family of Companies has a Full Time opening for a Director of Rehabilitation at Great Lakes Healthcare Center , where our therapists and Leaders are In-House Team Members . This is an exciting opportunity to make a direct impact with our residents through therapy. CommuniCare is a rapidly growing company , with 170 centers across IN, MD, MO, OH, PA, VA, WV

Requirements

Must be a state licensed clinician (PT, OT, SLP, PTA, or COTA) from an Accredited Program.
Excellent oral and written communication, and effective leadership skills

Responsibilities

Performs Evaluations, Screens, and treatments of facility patients/residents who are in need for Therapy services.
If an evaluating therapist, perform patient/resident evaluation under physician order and develops a treatment plan in accordance with patient’s needs.
Implements treatment plan through direct treatment, education of treatment staff which includes therapists and other members of team involved in the treatment plan.
Attends/leads interdisciplinary meetings as appropriate, as well as function as a resource for clinical programming and caseload.
Will also relay clinical findings and make recommendations to other members of the rehab team.
Collaborates with facility and corporate Denials Management Specialist regarding any ADR/denied therapy claims.
Conducts staff/family training and education when appropriate by formal and informal in-services as well as consultations.
Maintains accurate billing information in accordance with facility procedures daily, including training/re-training of team members, and complies with documentation requirements for rehab department.
