About The Position

Transitions Healthcare Autumn Grove Care Center is looking for an experienced Director of Rehab to organize and oversee all therapy treatment operations of the facility. You will create a collaborative and efficient workplace with attention to quality standards and policies while fostering an environment where all personnel will be able to do their best in caring for residents and their families. Being efficient, dedicated, a good listener and a problem-solver is what will open our door for you. Requirements

  • Licensed as a rehab therapist or therapy assistant (PT, PTA, OT, COTA, SLP) in the Commonwealth of Pennsylvania in good standing.
  • A minimum of three (3) years of rehabilitation experience.
  • Must be proficient with the MS Office Suite and Outlook.

Nice To Haves

  • Point Click Care experience preferred.

Responsibilities

  • Plan, organize and manage the effective and efficient delivery of rehabilitation services within the facility through the development of a viable interdisciplinary team/partnership.
  • Supervision of Department personnel including but not limited to orientation and training as well as scheduling and performance evaluations.
  • Review and update Department policy and procedures on a regular basis to ensure compliance with facility process and state/federal regulations.
  • Conduct monthly quality assurance reviews to include: chart audits on active and discharged residents, informing staff of deficiencies and chart completion as indicated.
  • Conduct quarterly program evaluation reviews identifying rehab program evaluation areas, collection of program evaluation information and development of action plan for non-conforming areas.
  • Prepare weekly management reports and monthly management reports outlining therapy productivity, indirect time analysis and action plan.
  • Review of referrals and discharges from rehab and Long Term Care restorative programming to evaluate service delivery and needs.
  • Attendance at facility meetings as needed/appropriate.
  • Assist in facility and rehabilitation marketing including referral rehabilitation screenings, community and referral source servicing, providing program evaluation and outcome information for inclusion in marketing materials and assisting in market analysis and provide recommendations for new rehabilitation programs.
  • Reviews and makes recommendations to the facility concerning staff education needs and requests.
  • Provides in-services and obtains speakers to assist staff in improving their job performance.
  • Provide direct resident care where needed and appropriate.
  • Attend in-service/educational programs in accordance with Company and regulatory requirements.
  • Ability to push/pull equipment weighing in excess of 50 pounds using proper techniques and with assistance as necessary.  Be able to sit, bend, and stoop.  Stand and/or walk for four (4) hours or more daily. Must be able to sit and keyboard for up to four (4) hours per day.  Reasonable accommodations may be made to enable individuals with disabilities to perform essential job functions.
  • Ability to read, write and understand the English language.
  • Assist in the evacuation of residents during emergency situations.
  • Meet general health requirements set forth by the policies of the Company, which may include a medical and physical examinations.
  • Perform other duties as directed by the Administrator
