The Director of Population Health is a strategic leader who will shape and grow Care Advantage's Value Based Care (VBC) programs. Reporting directly to the Senior Vice President of Value Based Care, this role is responsible for managing Managed Care Organization (MCO) relationships, driving program performance, and identifying opportunities to expand VBC revenue. This is a high-impact position where you'll collaborate across clinical, operational, and financial teams to ensure Care Advantage continues to lead in delivering measurable outcomes for patients and partners.

  • VBC Performance Management
  • Monitor ongoing performance metrics against contractual obligations and budget targets.
  • Develop standardized reporting packages for internal leadership and external partners.
  • Partner with CIO and Business Analytics teams to design new tracking tools and dashboards.
  • Provide oversight and guidance to VBC Care Coordinators, ensuring performance goals are met.
  • Launch targeted initiatives to improve patient outcomes and drive VBC metrics.
  • Set and achieve quarterly goals aligned with organizational strategy.
  • MCO Account Management
  • Serve as the primary point of contact for MCO program relationships.
  • Prepare and lead Operational Touchpoints and Steering Committee meetings.
  • Identify opportunities to expand contracts and strengthen partnerships.
  • Collaborate with Regional Operations and Clinical teams to support local initiatives.
  • Partner with Finance to calculate, track, and collect VBC invoice payments.
  • Escalate issues with MCO leadership to remove barriers and ensure program success.
  • Strategic Leadership
  • Act as a thought leader in value-based care, bringing innovative ideas to improve outcomes.
  • Collaborate with executive leadership to align VBC initiatives with company growth strategy.
  • Represent Care Advantage at industry forums, conferences, and partner meetings.
  • Bachelor's degree required; advanced degree preferred.
  • 5–10 years of experience in value-based care, managed care, home care, primary care, or similar healthcare roles.
  • Proven track record in account management and program performance evaluation.
  • Experience managing clinical resources such as nurses and care coordinators.
  • Strong communication, negotiation, and relationship-building skills.
  • Proficiency in Microsoft PowerPoint, Word, Excel, and Teams.
  • Ability to thrive in a fast-paced, collaborative environment.
