About The Position

The Director of Population Health & Quality is responsible for the strategic leadership, operational oversight, and performance outcomes of Cedar Point Health’s population health, quality improvement, and value-based care initiatives. This role ensures achievement of quality metrics across Medicare Advantage payees and accountable care partnerships, including Cedar Point Health’s collaboration with Aledade. Serving on the Leadership Team, they will support the efforts of and work collaboratively with the CEO and CMO to support the strategic objectives of the organization. This is an on site position.

Requirements

  • This position is required on site 3-4 days per week, minimum.
  • Ability to demonstrate flexibility and a positive attitude in a high-case load, rapidly changing priority work environment.
  • Ability to cultivate strong professional client-service rapport
  • Must be experienced in a physician’s office or equivalent combination of training and experience.
  • Must possess strong leadership and experience managing multi-disciplinary teams.
  • Undergraduate Degree in; Nursing, Public Health, Healthcare Administration, Business or a related field.
  • Clinical Licensure: RN, NP, PA or CSW required
  • 2+ years of supervision experience required
  • Must possess the physical and mental abilities that are mostly sedentary with occasional standing, walking, reaching.
  • Daily and repetitive data entry may cause nerve problems unless ergonomic techniques are used.
  • Periodic stress occurs from handling many calls and dealing with patient requests.
  • The position requires the ability to work under pressure and with a diverse population, including staff, physicians, clients, patients, insurance companies, labs, hospitals and other members of the public on a regular basis.
  • The position may cause exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinical environment.

Responsibilities

  • Serve on Leadership team to assist in strategically guiding the organization
  • Lead and supervise a team of employees, including Quality Coordinators, Chronic Care Managers, and Annual Wellness Visit Nurses
  • Conduct performance evaluations, professional development, timecard approvals and time off management.
  • Attend monthly staff meetings.
  • Promote a culture of accountability and continuous improvement
  • Develop and coach on foundational programs including patient messaging and quality outreach campaigns.
  • Design and implement population health strategies aligned with Medicare Advantage requirements,
  • Monitor quality measures including A1c, blood pressure, cancer screenings, statin use, and medication adherence.
  • Partner with primary care providers to close care gaps.
  • Lead quality improvement initiatives
  • Ensure compliance with payer and regulatory requirements
  • Oversee quality reporting dashboards, weekly
  • Work collaboratively with clinical and billing staff to ensure quality measure processes are being followed and data integrity requirements of our payer partners is being met through proper coding and billing.
  • Use population health tools including the electronic health record system (EHR)
  • Translate data into actionable improvement plans, collaborate with Leadership to implement.
  • Stay current on evolving ACO program requirements
  • Present performance updates to CPH Leadership Team, BOD and Owners.
  • Oversee chronic care management workflows
  • Collaborate with annual wellness visit nurses (AWV) to maximize preventative care delivery
  • Support patient engagement initiatives
