Requirements

  • RN with a baccalaureate or higher degree in nursing or a health‑related field AND 3 years of experience within the last 5 years in a home health agency, primary care clinic, or health facility, including at least 1 year in a supervisory or administrative role OR RN with 4 years of experience within the last 5 years in a home health agency, primary care clinic, or health facility, including at least 1 year in a supervisory or administrative role Note: “BSN preferred” language alone is not sufficient for CA if the degree‑based pathway is used, and the experience timeframes must align with the last 5 years.
  • CPR certification
  • DPCS has sufficient background knowledge and expertise in clinical decision‑making appropriate to the agency’s patient population and contributes to Quality Management / QAPI review and evaluation
  • Extensive knowledge of state and federal home health laws and regulatory standards
  • Excellent management, organizational, problem-solving, communication and interpersonal skills

Responsibilities

  • Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards
  • Coordinate and manage a team of nurses, therapists, CNAs, aides and social workers to ensure client and patient goals are met
  • Recruit, hire, orient, train, supervise and retain qualified clinicians and caregivers
  • Ensure medical supplies/equipment are delivered and staff are trained on care procedures
  • Assess patient/client needs, resolve issues, and ensure a high level of satisfaction
  • Effectively manage financial resources related to the delivery of quality patient/client care
  • DPCS will devote a sufficient number of hours to ensure the quality, adequacy, and supervision of patient care services.
