About The Position

The Terraces at Los Altos is a nonprofit Life Plan Community located in a quiet residential neighborhood near downtown Los Altos. As part of HumanGood, one of the nation’s largest nonprofit senior living providers, we are committed to inspiring the best life for residents and team members. We are seeking an experienced and compassionate Director of Nursing (DON) to lead the Skilled Nursing Health Center. This senior leadership role oversees all clinical operations, ensures regulatory compliance, and builds a high-performing nursing team while partnering closely with residents, families, and the interdisciplinary care team to deliver exceptional, person-centered care. $135K-$160K (DOE) plus performance-based incentives $15,000 sign-on bonus!

Requirements

  • Active California Registered Nurse (RN) license and graduation from an accredited nursing program.
  • Current CPR certification.
  • Bachelor’s degree in Nursing required; Master’s degree preferred, or a comparable blend of education and relevant professional experience.
  • 3–5 years of progressive nursing leadership experience, ideally in a Skilled Nursing Facility or related care environment.
  • Strong clinical assessment and critical‑thinking abilities, with a keen eye for detail and commitment to best‑practice standards.
  • Proficiency with electronic health records (EHRs) and comfort navigating computer‑based documentation, communication platforms, and reporting tools.

  • Master’s degree preferred, or a comparable blend of education and relevant professional experience.

Responsibilities

  • Conducting routine rounds throughout the Health Center to monitor resident well‑being, assess quality of care, and confirm that staffing patterns align with current resident needs and acuity levels.
  • Ensuring the delivery of comprehensive and appropriate nursing services consistent with resident care plans, regulatory standards, and evidence‑based practice.
  • Serving as a primary point of contact for residents, families, and staff, providing guidance, addressing concerns, and fostering a supportive and collaborative environment.
  • Overseeing admissions, transfers, and discharges, coordinating closely with partner hospitals and teams across Assisted Living, Memory Support, and Independent Living for seamless transitions of care.
  • Leading, supervising, and developing nursing and department leadership teams, including responsibility for hiring, onboarding, scheduling, coaching, performance evaluations, and addressing staff feedback and concerns.
